Co-developed by the Canadian Dental Association and clinician members of the Canadian Society for Disability and Oral Health, the Dental Treatment Case Complexity Form and Recommendations is an assessment tool to help dentists and the dental team in making referral decisions when seeing persons with special health care needs.
For patients and families/caregivers, a formalized transition process will help guide them through the transfer from pediatric to adult oral health care smoothly. For dentists and the dental team, following a step-bystep process will help ensure a more organized and consistent clinical protocol for facilitating transition preparation and patient transfer
Functional Assessment:
Pre-Visit Questionnaire and Supplemental Guide
The functional assessment form is designed for all clients in school and practice settings. The goal of the functional assessment is to proactively acquire information to facilitate the introduction of all clients into dental and dental hygiene care and avoid miscommunications that can negatively impact the building of trust. The information gained from this form is beneficial in preparing to care for all clients, such as those with dental anxiety and other special health care needs.

Home Dental Desensitization Kit
Desensitization is an evidenced-based method to introduce patients with sensory challenges to the dental environment.
Personalized kits containing pictures of the dental personnel the patient will meet, the office itself and the equipment that may be used are very helpful in introducing the items slowly and building confidence. Kits also include common tools that caregivers can use with the patient before an appointment to help the patient get used to what they will experience during a hygiene or dental appointment.
The Home Dental Desensitization Kit describes items that can be included in a kit, their purpose, and how to use them. Kits can be made with a few items in each so they are manageable for the caregiver and the patient. Click on the button to view the toolkit.
